Orthodontics & Porcelain Restorations

The ChallengeThe patient wanted to improve the alignment, shape, and appearance of the front teeth. Their existing tooth positions would have required more extensive preparation for veneers and crowns.

The Approach

Orthodontic treatment was completed first to reposition the teeth and create a better foundation for the final restorations. Porcelain restorations were then used to refine the shape, proportions, and appearance of the smile.

Why Orthodontics First?

Properly positioning the teeth before veneers & crowns can allow for a more conservative preparation and help preserve natural tooth structure. It can also provide a more predictable foundation for the final result.

The Result

The combination of orthodontics and porcelain restorations created a balanced, natural-looking smile while minimizing the amount of natural tooth structure removed.

An Important Limitation

Not every patient is a candidate for veneers without prior orthodontic or other treatment. Tooth position, bite, enamel thickness, gum tissue, and the desired result all influence treatment recommendations and sequence. In this case, the patient elected not to pursue additional treatment to reposition the gum tissue, so the final result reflects the natural limitations of the existing gum positions.

Restoring Health, Function, & Esthetics

The ChallengeThe patient presented with multiple misshapen and malpositioned teeth resulting from years of wear, fractures, and previously placed composite bondings and crowns. The goal was to create a more cohesive, natural-looking smile while addressing the existing damage and restorations.

The Approach

The affected teeth were carefully restored to improve their shape, proportions, position, and overall health & harmony. Existing restorations were replaced as needed. The lower natural teeth were whitened prior to selecting the final shade, allowing the restorations to complement the patient’s natural tooth color.

Why This Approach?

When multiple teeth have different histories of wear, fracture, and previous dental work, treatment must balance cosmetic improvement with the preservation of healthy tooth structure. The restorations were designed to work together while maintaining a natural appearance.

The Result

The completed treatment created a more uniform and balanced smile, restoring damaged teeth while improving their overall shape and appearance.

An Important Limitation

This case was influenced by existing bone loss and the need to maintain periodontal health. The patient also elected not to treat the lower teeth. As a result, the final esthetic outcome was limited and designed to blend with the natural lower teeth rather than create an isolated or overly uniform appearance. These considerations demonstrate how treatment recommendations and final esthetics must be individualized to each patient’s oral health, existing teeth, and treatment preferences.

I could do better with my oral care like flossing (which is very important) and she doesn’t make me feel bad about it. She encourages me to do better and is very gentle during my cleanings. Her reception staff, hygienist and assistants are more than competent, courteous, welcoming and professional.  And Dr. Gapstur is an amazing dentist! I have had extensive dental work done over the years and need additional work now. The first session I had for crowns and a filling was pain-free and very comfortable.
